Take a few extra minutes and go thru your book one last time!!

While your books are pretty much in order - it would not hurt to go through them one (1) last time to ensure you have every document that will be needed for the National Championships. The worse thing that could possibly happen is to have a missing document, birth certificate or signatures missing (parent and/or participant).

Team Books MUST Include the following Documents for Each Participant:

FOOTBALL:
a - Team Books MUST be Assembled in Numerical Jersey # Order with Older/Lighters First.
b - National AYF Mandatory Play Roster - Completely filled out and ready to use, 5 Copies.

CHEERLEADING:
b - Team Books MUST be Assembled in Birth Date Order Oldest to Youngest.

Remainder of all books both Football and Cheer MUST INCLUDE THE FOLLOWING:
     1) Official Roster Certified by the Conference (National Office for Wildcard Entries) 2 copies
     2) Participant Photo ID Contract Form
     3) Medical Clearance Form
     4) Proof of Age (Original Birth Certificate for Football; Cheer/Dance will accept a copy)
     5) Emergency Medical Treatment Consent & Information Form
     6) Absentee Form (if Applicable)

FOOTBALL:
All Teams must come to the tournament with both their Home and Away jersey. If an advancing team does not have 2 jerseys they must inform the National Office prior to arrival.

The following paperwork will be retained by the National office. These documents should be placed in a separate folder in the Team Book and ready to be turned over at Check-In.

A) Team Paperwork:
     1) Official Roster Certified by the Conference (National Office for Wildcard Entries
     2) Background Check Affidavit
     3) Scholastic Fitness Affidavit
     4) Proof of Insurance & Risk Management Agreement
     5) AYF/AYC Insurance Checklist
     6) Copy of Certificate of Insurance

B) Coach/Volunteer Paperwork: These documents must be in order as listed on your Certified Roster:
     1) Adult Waiver/Release Form (Adult)
     2) Adult Image Release Form (Adult)

C) Participant Paperwork: These documents must be in book order:
Football - Numeric Jersey # Order, Older/Lighter First
Cheerleading/Dance - Birth Date Order Oldest to Youngest
     1) Participant Waiver/Release Form (Minor)
     2) Participant Image Release Form (Minor)

NOTE: Falsifying birth or any other documents to make a participant eligible is grounds for forfeiture of all games in which they have participated and may result in the permanent suspension of the adult (s) and/or organization responsible for the act.

Do not forget to bring your entry fees to Team Check-In:
     Football - $150.00 per team
     Cheer Small (5 to 17) - $100.00 per team
     Cheer Large (18 to 35) - $200.00 per team.
